Survey Information: Surveys for US children in follow up
The PROSpect CCC (Clinical Coordinating Center) at the University of Pennsylvania will be sending the surveys for you to complete about your child during this study.
The PROSpect study team at your child’s hospital will help you complete the first survey. The CCC will send a link to the first survey and they will open it for you and briefly explain how to answer the questions.
The first survey will ask about your child’s overall health for the 7 days before their current illness.
About 2 weeks after your child leaves the PICU, the PROSpect CCC will contact you to confirm the best way to reach you for the remaining surveys. You will be asked to complete 4 surveys about how your child is doing.
Each survey can be completed electronically. We will send you a link by email using a software called REDCap. You can use a computer, tablet, or smartphone. If you are unable to access the link, you can complete the survey by phone with a member of the CCC PROSpect.
